Surgical myocardial revascularization on the beating heart termed off-pump coronary artery bypass (OPCAB) grafting predated coronary artery grafting with the assistance of cardiopulmonary bypass. After the advent of the heart-lung machine, OPCAB operations were virtually abandoned, with a few exceptions. Over the past two decades OPCAB has been reinvented predominantly due to the realization that surgery on cardiopulmonary bypass may be associated with unwanted side effects that may result in suboptimal outcomes for the increasingly high-risk patient groups currently being referred for coronary artery bypass surgery. This chapter presents the historical background of the development of OPCAB. © 2012 Nova Science Publishers, Inc. All rights reserved.
