The logistics of vaccination programs are considered complex, as the supply chain depends on multiple factors, including how the vaccine is stored and how it will be distributed, according to aspects of the region and demands. The general objective of this study is to analyze and expose how the logistical distribution of vaccines takes place in regions with difficult access in the city of São Paulo, and the specific objectives are: to evaluate the logistical process of vaccine distribution in different regions of the city; observe the main difficulties that patients face to use the vaccination service; map the logistical flow; explore transportation alternatives to regions with difficult access. For the analysis, data were collected through the application of questionnaires with samples from three different groups: general population, healthcare professionals and logistics specialists. Areas of difficult access were considered to be the regions with the highest number of slums and rural households, being them East, South and North. Through the evaluation of the questionnaire results, it was observed that, in general, all regions of the city have health units that participate in the national immunization program and that they store the materials properly, guaranteeing the population's vaccination. Even so, some flaws, such as delays in the delivery of vaccines and improvements, especially in the infrastructure of health facilities, were highlighted by respondents.
