Objective: The purpose of classifying the fractures is to indicate treatement, as well as compare its results and estimated prognosis. Several classification systems have already been described none of which, however, is generally accepted. Two of them are most used: Alle and AO. The authors aim to compare the reliability of these classifications. Methods: It was used X-rays images of 27 patients with lower cervical spinal fractures, which were classified by ten surgeons. In three cases the analysis was repeatedto asses the intraobserver reliability. Results: By matching the answers with the Kappa coefficient it was found an interobserver and intraobserver reliability of 0.49 and 0.70 for the Allen classification, respectively, and of 0.41 and 0.61 for the AO classification. Conclusion: Both classification systems showed good agreement in the intraobservers evaluation, and fair in the interobservers; also, Allen classification had a better agreement than AO.
